Unlock Everyting
At the options screen, hold L1 + L2 + R1 + R2 and press X, Up, X, Up, Down, Square to unlock everything.

Unlock Extra Option
Complete and save the game using the 'normal' difficulty setting to unlock the 'Extra' option at the main menu.

Unlock Extra 2 Option
Complete and save the game using the 'hard' difficulty setting to unlock the 'Extra 2' option at the main menu.
