When at the title screen, reset your console 10 times. Hold the Left/Down diagonal while entering the options menu for a level select.

Twenty Continues
Pause the game and press Up, Up, Up, Down, Down, Left, Left, Left, Right, Right, C, C, B, A, Start for 20 continues.
